Coaches Packet

USA HOCKEY REQUIREMENTS MUST BE COMPLETED BEFORE PARTICIPATING IN ANY TEAM ACTIVITIES

USA Hockey #’s -  Coaches are required to register every season

CORi Forms- Coaches are required to complete an updated MA HOCKEY CORI form every     season.

  • Must be completed before taking ice.

  • Valid Driver’s License/ ID is required

  • Form and Identification must be submitted in person to Jessica Cusumano

  • Copy is provided with this packet

USA Hockey Safe Sport Training- required every two years

  • Must be completed before taking the ice

  • Completed Certificates MUST be e-maied to registrar@capeannyouthhockey.com

  • You will need a current USA Hockey Number to register

  • Visit http://www.usahockey.com/safesporttraining

  • If you took the course in 2016-17 it was valid for the 2016-17 and 2017-18 season and you WILL NEED TO COMPLETE the SafeSport Refresher course for the 2018-19 season.

  • If you took the course in 2017-18 you are up to date and do not need to complete the refresher until the 2019-20 season.

CEP class list and info -Must be registered for a Clinic no later than 10-31-2018 (email Mandy with class date) and must be completed prior to 12-31-18

Coaches MUST attend a yearly CEP clinic prior to December 31st of the current playing season. Mite (8 & under) coaches may remain at Level 1 (even if it is expired) until they move to another age level of play.

If you currently do not have a CEP certification you must register for Level 1

If you are Currently Certified with:

Level 1 you must complete Level 2

Level 2 you must complete Level 3

Level 3 you must complete the Level 3 recertification (track 1 or track 2) or attend a Level 4     clinic

Level 3 plus Track 1 you must recert with Track 2 or attend a level 4 clinic

Level 3 plus recert of track 1 and 2 you must complete Level 4 certification

  • Cost is $40.00

  • Visit: http://www.usahockey.com/coachingclinics for list of available dates. The coaching clinic season for levels 1-3 officially runs from September 1 to December 31.

  • Reimbursement is available if you email your receipt to

  • If this is not completed by 12/31 you will no longer be able to coach

Age Specific Modules

  • Coaches are required to complete the online age-specific module(s) for the age level of play you are coaching

  • Coaches must have current USA Hockey Number

  • Online modules are available September 1st

  • Cost is $10.00

  • Completed Certificates MUST be e-maied to registrar@capeannyouthhockey.com

  • Reimbursement is available if you email your receipt to

  • Note after December 31st you are subject to a increased cost of $50.00 if not completed by 12-31. Reimbursement is not eligible.

  • If your module is not completed by 12-31 you will no longer be able to coach

Procedure for injuries and insurance

In the event of any accident or injury, a report should be filed immediately.
